The 'Community Hero'

Heroic Individual

This award recognises individuals who have made a real difference to the lives of others. For example: a blogger, a group organiser, a fundraiser, an activist or a special carer. This award is open to any heroic individual that does not make their primary income in this area.

If you are an individual who makes your primary income in the area of mental health then we ask you to see our 'Professional Hero' award category. If you are a musician, writer, artist, we ask that you look at the 'Creative Hero' category. Please only enter one category per nominee.

Heroic Organisation

This award recognises organisations that have contributed to the field of mental health, either by raising awareness, educating or directly supporting those with mental illness. This award is open to any non profit making organisation, whether charities, social enterprises, community interest projects or community groups. We want to hear from any groups that have made an impact on lives in the community, if you are a business or your organisation is for-profit, we still want to hear from you, please see our 'Business Hero' award category.

Terms

As the organisations involved in these awards work in the mental health community and touch the lives of 'heroes' every day, it would be folly for us not to accept entries from people who have had contact with any of us. However, in the interest of fairness there are a few rules we ask you to adhere to.

1) Fantelli Imprint, SANE, Mental Health Foundation, The Centre for Mental Health and Emergence employees are NOT eligible to apply. Volunteers/fundraisers and/or people who have worked with the organisations but are NOT employed by any of them ARE eligible to apply.

2) If you know any of the judges or organisers personally (in this we mean have contact outside a working capacity) you are still eligible to apply, but you MUST state this on your entry form so we can ensure you are judged by three impartial judges.

3) Each nominee should be entered into just one category. You can enter an organisation AND an individual that works within the organisation in different categories, but each organisation/individual should only be nominated in one category each. You can nominate more than one candidate. We invite all nominees to nominate a celebrity creative that has inspired them in the 'Creative Hero' - professional category.

4) Nominations close on 10th December 2011, entries made after this time cannot be counted as judging will start immediately. Shortlisted candidates will be contacted via email sometime during the week commencing 19th December. The shortlist will be published on the website www.mentalhealthy.co.uk the same week.

5) You agree that the name of the nominee will be published on the website and in any promotional material should they make the shortlist.
